My daughter's grandparents wish to adopt her and I am trying to find a form in which I can just have visitation at least twice a year.

You can use the Parenting Plan form found on the Florida Courts website. Because the grandparents are the petitioning party, they will be the ones to fill it out and get your consent and waiver or you can have the court approve it at a hearing.
