Filing to Have Ex-Husbands Rights Dissolved

I am the mother of an 8 year old daughter. I have raised her solely since she was 2. The judge gave my x-husband supervised visits because of the proven drug use around my child. Six years later he is in jail for drugs and hasn't paid child support in years.

I want to file to have his rights dissolved all together. What paperwork do I need to start this process and file on my own?

The form you need to start this process is the Supplemental Petition to Modify Parenting Plan/Time-Sharing Schedule and Other Relief. You will be asking the court to establish no contact and in the "other relief" section you will be asking the court to remove parental rights.

What you are attempting will be difficult to do on your own. You might want to seek legal advice, especially if the father of the minor child contests this. Only you can decide what's best.
